Altin-Met is a prescription combination tablet containing vildagliptin and metformin hydrochloride for improving blood-glucose control in adults with type 2 diabetes mellitus. It is used with dietary management and physical activity, alone or with other antihyperglycemic treatment when prescribed.

  • Vildagliptin may cause elevated liver enzymes; rarely, hepatitis or other liver injury may occur.
  • Rare but serious effects include pancreatitis, severe allergic reactions, skin or blistering reactions, and metformin-associated lactic acidosis.
  • Report severe or persistent abdominal pain, yellowing of the skin or eyes, facial swelling, breathing difficulty, or symptoms of lactic acidosis immediately.

Contraindications

  • Do not use in patients with allergy to vildagliptin, metformin, or any tablet ingredient.
  • Contraindicated in diabetic ketoacidosis, diabetic precoma, or metabolic acidosis.
  • Do not use in severe kidney impairment or when kidney function is unsuitable for metformin therapy.
  • Generally contraindicated in significant hepatic impairment or active liver disease; liver function should be assessed as clinically appropriate.
  • Use is unsuitable in acute conditions that may cause dehydration, severe infection, hypoxia, heart or respiratory failure, or reduced kidney function.
  • Temporarily discontinue around iodinated contrast procedures or major surgery when advised by the treating clinician.
  • Seek urgent medical advice for persistent vomiting, severe abdominal pain, unusual muscle pain, marked weakness, breathing difficulty, or extreme drowsiness.

Precautions & Warnings

  • Alcohol increases the risk of metformin-associated lactic acidosis and should be avoided or minimized.
  • Iodinated contrast agents may worsen kidney function and increase lactic-acidosis risk; metformin may need to be withheld before and after contrast administration.
  • Insulin, sulfonylureas, and other glucose-lowering medicines may increase the risk of hypoglycemia; dose adjustment may be required.
