- Not intended for the treatment of type 1 diabetes or diabetic ketoacidosis.
Usage & Safety
For Adult
- Take one tablet once daily, or use exactly as prescribed by the physician.
- Swallow whole with water; it may be taken with or without food.
- Maintain adequate fluid intake unless fluid restriction has been advised.
- Dosage may require adjustment or discontinuation during acute illness, dehydration, surgery, or significant kidney impairment.
For Children
- Safety and effectiveness for children and adolescents under 18 years have not been established for this fixed-dose combination.
- Not routinely recommended for pediatric use unless specifically prescribed by a specialist.
- Do not use in children or adolescents with type 1 diabetes or suspected diabetic ketoacidosis.
Side Effects
- Common effects may include increased urination, thirst, genital fungal infections, urinary tract infections, nausea, or nasopharyngitis.
- Low blood sugar is more likely when used with insulin or sulfonylureas.
- Seek urgent medical attention for symptoms of ketoacidosis such as nausea, vomiting, abdominal pain, unusual tiredness, breathing difficulty, or confusion, even if blood glucose is not very high.
- Seek medical attention for severe allergic reactions, facial or throat swelling, severe rash, or difficulty breathing.
- Rare but serious effects may include severe dehydration, kidney problems, pancreatitis, or serious genital or perineal infection.
Contraindications
- Do not use in patients with a known allergy to empagliflozin, linagliptin, or any tablet ingredient.
- Contraindicated in diabetic ketoacidosis or acute metabolic decompensation of diabetes.
- Not indicated for type 1 diabetes mellitus.
- Avoid or use only under specialist supervision in severe kidney disease, dialysis, marked dehydration, or recurrent urinary or genital infections.
- Use caution in patients with a history of pancreatitis, low blood pressure, heart failure, or conditions causing fluid loss.
Precautions & Warnings
- Insulin and sulfonylureas may increase the risk of hypoglycemia; the prescriber may need to reduce their dose.
- Diuretics and other medicines that lower blood pressure may increase dehydration, dizziness, or low blood pressure.
- Strong enzyme inducers such as rifampicin, carbamazepine, phenytoin, or phenobarbital may reduce linagliptin effectiveness.
- Tell the physician about all diabetes medicines, kidney-affecting medicines, lithium, diuretics, and blood-pressure medicines.
- Alcohol may increase dehydration and may contribute to poor diabetes control; avoid excessive alcohol intake.
Pregnancy
- Use during pregnancy only when specifically prescribed; empagliflozin is generally avoided, particularly during the second and third trimesters.
- Inform the physician immediately if pregnancy occurs or is planned.
- Breastfeeding is generally not recommended during treatment because potential effects on the nursing infant cannot be excluded.
