Ingredients
- Empagliflozin 25 mg
- Linagliptin 5 mg
- Metformin hydrochloride extended-release 1000 mg
Benefits
- Improves blood-glucose control in adults with type 2 diabetes mellitus.
- Used as an adjunct to an appropriate diet and regular exercise.
- May be prescribed when treatment with metformin and/or other glucose-lowering medicines does not provide adequate control.
- Empagliflozin may provide cardiovascular risk-reduction benefits in appropriately selected adults with type 2 diabetes and established cardiovascular disease.
Usage & Safety
For Adult
- Take exactly as prescribed by the healthcare professional, usually once daily with a meal.
- Swallow the tablet whole with water; do not crush, split, or chew the extended-release tablet.
- The selected strength should be based on the patient's current treatment, blood-glucose response, kidney function, and tolerability.
- Maintain adequate fluid intake unless fluid restriction has been advised by a healthcare professional.
For Children
- Not recommended for children or adolescents under 18 years because safety and effectiveness have not been adequately established.
- Not indicated for the treatment of type 1 diabetes mellitus or diabetic ketoacidosis at any age.
- No pediatric weight-based dosing recommendation is established for this combination.
Side Effects
- Common effects may include increased urination, thirst, genital fungal infections, urinary tract infections, nausea, diarrhea, abdominal discomfort, and reduced appetite.
- Metformin may cause gastrointestinal upset and, with long-term use, reduced vitamin B12 absorption.
- Low blood sugar is more likely when used with insulin or a sulfonylurea.
- Serious reactions may include diabetic ketoacidosis, lactic acidosis, pancreatitis, severe dehydration, acute kidney injury, serious urinary or genital infections, Fournier's gangrene, angioedema, and severe skin reactions.
- Seek urgent care for severe weakness, vomiting, abdominal pain, rapid breathing, confusion, fainting, facial or throat swelling, or painful swelling and infection around the genital or perineal area.
Contraindications
- Do not use in patients with a known allergy to empagliflozin, linagliptin, metformin, or any product ingredient.
- Contraindicated in diabetic ketoacidosis, diabetic pre-coma, or severe acute illness associated with metabolic instability.
- Avoid in severe kidney impairment or substantially reduced kidney function; kidney function should be assessed before and during treatment.
- Use is generally unsuitable in severe liver impairment, severe dehydration, or conditions associated with tissue hypoxia or unstable heart or breathing failure.
- Temporarily discontinue before iodinated contrast procedures, major surgery, or prolonged fasting when advised by a healthcare professional.
- Discontinue and seek urgent medical attention if symptoms suggest lactic acidosis, ketoacidosis, pancreatitis, or a severe allergic reaction.
Precautions & Warnings
- Insulin and sulfonylureas may increase the risk of hypoglycemia; dose adjustment and glucose monitoring may be required.
- Diuretics and medicines that lower blood pressure may increase dehydration, low blood pressure, and kidney-related risks.
- Alcohol may increase the risk of metformin-associated lactic acidosis and should be limited or avoided, particularly with binge drinking or poor food intake.
- Iodinated contrast agents may increase the risk of kidney injury and lactic acidosis; metformin-containing treatment may need to be withheld around contrast procedures.
- Rifampicin and other strong P-glycoprotein or CYP3A4 inducers may reduce linagliptin exposure and glucose-lowering effect.
- Cimetidine, dolutegravir, ranolazine, trimethoprim, corticosteroids, sympathomimetics, NSAIDs, ACE inhibitors, and other glucose-lowering medicines may require monitoring or dose adjustment.
- Tell the healthcare professional about all prescription medicines, over-the-counter products, supplements, and herbal medicines before starting treatment.
Pregnancy
- Not generally recommended during pregnancy, particularly during the second and third trimesters, unless the expected benefit clearly outweighs potential risks.
- Contact the prescribing healthcare professional promptly if pregnancy occurs.
- Empagliflozin may affect the developing fetal kidney; an alternative treatment may be preferred during pregnancy.
- Breastfeeding is generally not recommended during treatment because the effects of empagliflozin and linagliptin on the breastfed infant are uncertain and metformin is excreted in breast milk.
