Benefits
- Improves blood glucose control in adults with type 2 diabetes mellitus when diet and exercise alone are insufficient.
- May be used when treatment with the individual components is appropriate.
- Empagliflozin may provide additional cardiovascular and kidney-related benefits in selected patients, as determined by a healthcare professional.
Usage & Safety
For Adult
- Take one tablet once daily with a meal, preferably in the morning, or as directed by the prescriber.
- Swallow whole with water; do not crush, split, or chew the extended-release tablet.
- Dosage should be individualized according to blood glucose control, kidney function, tolerability, and previous therapy.
- Maintain adequate fluid intake unless fluid restriction has been prescribed.
For Children
- Safety and effectiveness in children and adolescents under 18 years have not been established for this fixed-dose combination.
- Not recommended for routine pediatric use unless specifically prescribed by a specialist.
- No pediatric weight-based dosing recommendation is established for this product.
Side Effects
- Common effects may include nausea, diarrhea, abdominal discomfort, reduced appetite, increased urination, and mild dehydration.
- Genital fungal infections and urinary tract infections may occur, particularly with empagliflozin.
- Low blood sugar is more likely when used with insulin or sulfonylureas.
- Seek urgent medical care for symptoms of ketoacidosis, severe weakness, vomiting, abdominal pain, rapid breathing, confusion, or unusual drowsiness.
- Seek immediate care for severe allergic reactions, facial or throat swelling, difficulty breathing, severe persistent abdominal pain, or signs of lactic acidosis.
- Rare but serious risks include severe dehydration, kidney injury, serious genital or urinary infections, pancreatitis, and lactic acidosis.
Contraindications
- Do not use in patients with a known allergy to empagliflozin, linagliptin, metformin, or any component of the tablet.
- Contraindicated in severe kidney impairment, dialysis, or conditions with substantially reduced kidney function.
- Do not use in metabolic acidosis, including diabetic ketoacidosis, or lactic acidosis.
- Use is not appropriate for treatment of type 1 diabetes.
- Use caution or avoid during severe dehydration, serious infection, shock, acute heart failure exacerbation, or conditions causing tissue oxygen shortage.
- Temporarily discontinue around iodinated contrast imaging when clinically indicated and restart only after kidney function has been reassessed.
Precautions & Warnings
- Insulin and sulfonylureas may increase the risk of hypoglycemia; dose adjustment may be required.
- Diuretics and other medicines that lower blood pressure may increase dehydration or low blood pressure.
- Iodinated contrast agents may increase the risk of kidney injury and metformin-associated lactic acidosis.
- Excessive alcohol intake increases the risk of lactic acidosis and should be avoided.
- Medicines that affect kidney function or metformin elimination require medical review.
- Strong enzyme inducers, such as rifampicin, may reduce linagliptin effectiveness.
- Tell the healthcare professional about all prescription medicines, over-the-counter products, and herbal supplements before use.
Pregnancy
- Use during pregnancy is not generally recommended unless clearly prescribed after an individual medical assessment.
- Empagliflozin is generally avoided during the second and third trimesters because of potential effects on the developing fetal kidneys.
- Inform the prescriber immediately if pregnancy occurs or is planned.
- Breastfeeding is generally not recommended during treatment because empagliflozin and metformin may pass into breast milk and the effects on the infant are not fully established.
