Benefits
- Improves blood-glucose control in adults with type 2 diabetes mellitus.
- Used as an adjunct to diet and exercise.
- May be used alone or with other glucose-lowering medicines when prescribed by a healthcare professional.
Usage & Safety
For Adult
- The usual dose is one tablet once daily, taken at approximately the same time each day.
- May be taken with or without food and swallowed whole with water.
- Maintain adequate fluid intake unless fluid restriction has been advised.
- Dosage and suitability should be assessed by a physician, particularly in patients with impaired kidney function or those taking insulin or sulfonylureas.
For Children
- Not recommended for children and adolescents under 18 years because safety and efficacy have not been adequately established.
- No pediatric weight-based dose is established for this combination.
Side Effects
- Common effects may include increased urination, thirst, genital yeast infection, urinary tract infection, and mild gastrointestinal symptoms.
- Low blood sugar may occur, particularly when used with insulin or sulfonylureas.
- Serious warning signs include severe dehydration, dizziness or fainting, reduced urine output, severe abdominal pain, persistent vomiting, breathing difficulty, confusion, or unusual tiredness.
- Seek urgent care for symptoms of ketoacidosis, severe allergic reaction, pancreatitis, or a serious genital or urinary infection.
Contraindications
- Do not use in patients with hypersensitivity to empagliflozin, linagliptin, or any formulation ingredient.
- Not indicated for treatment of type 1 diabetes or diabetic ketoacidosis.
- Avoid use in patients receiving dialysis or with severe kidney impairment when glycemic benefit is not expected.
- Use caution in patients with a history of pancreatitis, recurrent urinary or genital infections, dehydration, low blood pressure, or significant liver or kidney disease.
- Temporarily discontinue and seek medical advice during prolonged fasting, major surgery, or serious acute illness when ketoacidosis risk may increase.
Precautions & Warnings
- Insulin and sulfonylureas may increase the risk of hypoglycemia; dose adjustment may be required.
- Diuretics and other medicines that lower blood pressure may increase dehydration or low blood pressure.
- Strong enzyme inducers such as rifampicin may reduce linagliptin exposure and glucose-lowering effect.
- Other medicines affecting kidney function or fluid balance require medical review.
- Alcohol may increase dehydration and may contribute to metabolic complications; avoid excessive alcohol intake.
Pregnancy
- Use during pregnancy is generally not recommended unless specifically prescribed after medical assessment.
- Empagliflozin should generally be avoided during the second and third trimesters because of potential effects on the developing kidneys.
- Breastfeeding is generally not recommended during treatment because it is unknown whether the medicine passes into human milk and potential infant kidney effects are a concern.
- Inform the doctor if pregnant, planning pregnancy, or breastfeeding.
