Usage & Safety

For Adult

  • Take exactly as prescribed, usually once daily at the same time, with or without food.
  • For hypertension or angina, treatment may commonly start at 5 mg once daily and may be adjusted according to response and tolerability.
  • Some patients may require a lower starting dose, particularly older adults or those with liver, kidney, or cardiac impairment.
  • For chronic heart failure, treatment is normally started with a much lower dose and increased gradually under specialist supervision; do not use the 5 mg strength to start or adjust therapy without medical advice.
  • Do not stop treatment suddenly, as this may worsen angina or cause serious cardiac complications.

For Children

  • Safety and effectiveness in children and adolescents have not been adequately established.
  • Not routinely recommended for patients under 18 years unless specifically prescribed by a pediatric or specialist physician.
  • No established weight-based pediatric dose is available for this product.

Side Effects

  • Common: tiredness, dizziness, headache, cold hands or feet, slow heartbeat, low blood pressure, nausea, diarrhea, or constipation
  • May cause sleep disturbance, depressed mood, or reduced exercise tolerance
  • May worsen breathing difficulty or cause bronchospasm, particularly in susceptible patients
  • Serious: fainting, very slow or irregular heartbeat, worsening heart failure, severe wheezing, swelling of the face or throat, or signs of a serious allergic reaction
  • Seek urgent medical care for chest pain, severe shortness of breath, or loss of consciousness

Contraindications

  • Allergy to bisoprolol or any component of the tablet
  • Very slow heart rate, symptomatic bradycardia, or severe low blood pressure
  • Cardiogenic shock or acute, decompensated heart failure
  • Second- or third-degree heart block, sick sinus syndrome, or significant conduction disease without a functioning pacemaker
  • Use with caution in asthma or chronic obstructive lung disease, diabetes, peripheral circulation problems, thyroid disease, or severe liver or kidney impairment
  • Medical assessment is required before use during acute worsening of heart failure or before surgery

Precautions & Warnings

  • Other medicines that slow the heart or lower blood pressure, including verapamil, diltiazem, amiodarone, digoxin, and some antiarrhythmic medicines, may increase the risk of bradycardia, heart block, or low blood pressure.
  • Clonidine and similar medicines may cause excessive slowing of the heart or rebound blood pressure changes if stopped improperly.
  • Insulin and other diabetes medicines may increase the risk of low blood sugar, while bisoprolol may mask warning symptoms such as a fast heartbeat.
  • Some antidepressants, antipsychotics, antihypertensives, and medicines affecting liver enzymes may increase its effects.
  • Nonsteroidal anti-inflammatory drugs, such as ibuprofen or diclofenac, may reduce the blood-pressure-lowering effect in some patients.
  • Alcohol may increase dizziness or low blood pressure. No specific food restriction is generally required, but take the tablet consistently with regard to meals.
